Pulse has an exciting and rewarding, opportunity for Community Psychiatric Nurse (CPN) to work as a HIU Coordinator in Walthamstow, to reduce attendances to hospital by care planning intervention. (nearest station Walthamstow Central rail and Underground or Wood Street Rail Station).

The Community Psychiatric Nurse (CPN) required to undertake a review of high Intensity user demand in the local ambulance service in addition to the A&E, police, Urgent Care Centres. The post holder is responsible for creating an innovative way of supporting High Intensity Users of ambulance 999 services, A&E and non-elective admissions. They will facilitate discussions and advise colleagues as to how best practice might be adopted for future processes and oversee their delivery.

You will be working an average of 37.5 hours per week, 09:00-17:00 Monday - Friday.

Requirements
* Post registration experience with significant years of working with adults with mental health problems and a minimum 6 months of working with adults in a community setting.
* Specialist skills in mental health assessment, risk assessment, safety planning and medication administration.
* Car owner/driver would be ideal but not essential as it is about finding the right person
* Coordination, sharing and learning of the work with community- based staff to promote safe practice and sustainability.
